Across the 41 programs in this index, the gap between the advertised price and the billed total comes from five recurring devices: mandatory memberships ($25–$145/month in our data — Mochi $79, IVIM $75, Eden $99, LifeMD↗ $149, Ro $145), starter-dose teasers, prepaid rates quoted as monthly prices, dose-change repricing, and onboarding extras like labs or "provider fees." Our all-in figures fold the first device in automatically; this guide is the field manual for the other four.

Memberships deserve one fair sentence before the knives come out: some buy real things. Mochi's $79 includes unlimited video visits with physicians and dietitians; Ro's $145 funds insurance navigation that can be worth thousands if it lands coverage. The problem is never the fee — it is the fee's absence from the headline. A "$99/month semaglutide" claim that is actually $178 once the mandatory membership joins is not a price; it is the first half of a price. Our basis rule exists for exactly this: every ranked figure on this site is medication plus every mandatory recurring fee, which is why Mochi appears at $178 and $278 here regardless of what the banner ad says.
The other devices, quickly. Prepaid-as-monthly: "$186/month" can mean $2,232 leaving your account today — a fine deal at a provider with clean refund terms, a trap anywhere the refund clause is vague; always ask for the number charged today. Dose-change repricing: tiered programs re-bill at every titration step, so demand the 10 mg and 15 mg figures in writing (full treatment here). Onboarding extras: intake "provider fees," required lab panels, and expedited-shipping charges are individually small and collectively a month's medication; programs in our index marked "shipping included" and "labs not required" — NexLife↗, Henry, OrderlyMeds↗ among them — remove the whole category. And one exit-side device: cancellation friction. A membership that recurs until cancelled, attached to a portal where cancellation requires a support ticket, is a pricing device too.
